Our Mission Is to increase the employable skills of young adults, displaced Individuals and those seeking additional knowledge in our community. ADMISSION REQUIREMENTS
New Applicants:

Will receive a complete enrollment packet after meeting with an enrollment counselor and/or Director of PSBC.

Proof Of Age:

All applicants must be 17 years of age for admission consideration. Proof of age is required when enrolling. Acceptable forms of proof of age are birth certificates, driver's license, passport and any other proof of as deemed appropriate and approved by the Director.

High School Diploma or GED:

High School transcripts or GED Is required for proof of education.

Transfer Students:

All transfer students to PSBC must submit an official transcript to the admissions office. The admissions team will evaluate all transcripts. At least 10% of the hours required for the certificate program must be completed at PSBC. In addition a practical and written examination will be administered. Upon approval of the transfer credits to PSBC by the NY State Education Bureau of Proprietary School Supervision, the new enrollee may register for his/her classes. Students who attended other cosmetology or technology school, college or university must complete an enrollment application. Financial aid, scholarships and/or assistance awards will be reviewed only after all required Information is received in the Admissions and Financial Aid offices.

All re-admlsslon applicants must adhere to the curriculum requirements at the time of re-admlsslon. An application to readmit must be submitted to PSBC admissions office.

FINANCIAL AID
Students attending PSBC must make their own financial arrangements to meet their financial obligation. PSBC is currently ineligible to accept state and federal education grants and loans. However, during the application process, eligible students may make arrangements with the P5BC Director to pay tuition through monthly installment plans. PSBC will also make available to the student any scholarship opportunities and additional information on available financing during the application process.

ADMISSION PROCEDURE AND ENTRANCE DATES
A prospective student should call the school to schedule an appointment for a personal interview. During the interview, prospective students will receive information regarding tuition, school policies, tour of the facility and fill out an application for admissions.

If a prospective student would like to enroll and is accepted into the school, the applicant will sign the Enrollment Agreement and receive a copy after a start date is agreed upon and the $100.00 registration fee is paid. Proof of age, and High School Diploma required. Any person seeking admission should be aware that the course requires a high level of manual dexterity and coordination and that there are extended periods of required stand up practical work required. Classes start the first Tuesday of every month.

The cost to attend (COA) PSBC includes tuition, supplies, equipment and transportation. Personal, room and board allocations will not be offered during the first year of operation.

Tuition is $5,000

Registration Fees are $100.00

Books/Equipment are $500.00

Full Time - 32.5 hours per week for 31 weeks (2 Terms)
Tuesday-Friday 9:00 am to 4:30 pm
Saturday 8:30 am to 1:00 pm
Lunch: 12 noon - 12:30 Tuesday - Friday (full time status only)

Part Time Evening Session - 19.5 hours weekly for 52 weeks (3 Terms)
Tuesday - Friday 6:00 p.m. To 10:00 p.m.
Saturday - 9:00 a.m. - 12:30 p.m.

Course Performance objective
The program is designed to prepare and educate the student for entry into the beauty Industry. PSBC program Is designed to ensure that every student will receive the necessary skills and tools required to graduate and compete in the cosmetology Industry. Each student will be prepared to successfully pass the NYS Board examination. Finally, students will learn the relevant skins of today with an emphasis on cutting, coloring, foil highlighting and professional conduct.

Dress Code
PSBC requires all students to adhere to the following professional dress code:

- A lab coat as approved by PSBC

- Black slacks or skirt (Knee length)

- Solid-Plain colored tops

- Black shoes (rubber sole) all shoes must be closed in

- No jeans, exercise attire, sneakers or sandals allowed

- No head wraps or scarves allowed unless authorized by PSBC personnel office

Access to Student File
PSCB will retain student files for twenty (20) years. A student, parent/guardian of a dependent student may view their records upon request and approval from the Academic office. except for the NAACAS, NYS Department of Education, US Department of education and they NYS Board of Licenses and authorized PSBC staff, PSBC will NOT release Information regarding its students to a third party, without a written consent form from the student. In the case of a minor such consent must come from the parent or legal guardian.

Progress Reporting and Grading:
Satisfactory progress in academic performance and attendance is required to continue as a student in good standing.

The academic grading system is as follows: monthly exams are administered and graded as follows:

A+ = 95-100
A = 90-94
B+ = 85-89
B = 80-84
C+ = 75-79
C = 70-74
D = 65-69
F = below 65

Students are graded monthly according to the results of written and practical examinations. Practical evaluation takes into consideration performance and . production. These grades are recorded on Progress Reports which are computed weekly.

To remain In good academic standing, a student must maintain at least a 70% average and complete the course in the designated time frame. A student who falls below a 70% average will have a conference with the Director before continuing the course. Any student failing to maintain at least a 70% average \n the academic and practical segments Is subject to probation. During probation the student's performance will be monitored and additional academic assistance will be given in an effort to assist the student to be removed from probation, the student must correct the deficiency that caused the probation and must do so before the next evaluation period. evaluations will be conducted 30 days following the last grading period. Failure to improve during a probationary period will subject the student to dismissal. Both are reviewed at the 30-day interval following the last grading.

Attendance/Absences
excessive absences and tardiness will directly affect your grades. A 20% absence rate in the 1st half of the coutse will result in dismissal. Excused Absences
Funerals, Medical procedures, jury duty, military leave and prior excused absences as authorized by PSBC Management staff, are the only acceptable forms of excused absences. However, students are required to make up the missed training and/or examinations.

Leaves of Absences (LOA)
PSBC Director must approve an LOA. The student must be in good academic standing at the time leave Is granted. All requests for LOA must be in writing and submitted to the Director's office. LOA are available from the administrative staff and are granted at the discretion of the Director. If approved, the student will not accumulate absent hours or received credit during the LOA. A LOA consists of a minimum of one week and a maximum of 60 days. A student is allowed one leave of absence per 12-month period. Students who fail to return after the last day of the leave will be terminated.

Upon. return from a LOA student's retention level will be assessed through one of three means: Counseling session with the student; Employing a written evaluation; or Oral evaluation.

PSBC RULES & REGULATIONS
1.) all students must adhere to the dress code as outlined above.
2.) All absences must be reported to the office with explanation for absence . (Supporting documentation i.e. Doctor's slip etc. May be requested
3.) Emergency calls as qualified by PSBC can be made at designated phones.
4.) Cell phones and pagers are to beturned off during class (Okay for use during breaks).
5.) Person hygiene and good grooming must be maintained in the classroom and clinic including use of deodorants, light fragrances and fresh breath.
6.) Children, friends and/or relatives are not allowed in the facility during class sessions.
7.) Seniors must be prepared for State Board as announced by management, with their own supplies.
8.) The intake staff assigns patrons, you are not allowed to switch or refuse patrons.
9.) Please exercise ethical code of standards and hospitality when greeting patrons.
10.) Please greet your patron (pleasantly), make sure they are comfortable, draped secured properly and all necessary prep work is implemented.
11.) Please review the customer slip for services and review with patron.
12.) Outside products brought in by the patron cannot be administered at PSBC.
13.) Upon completion of your patron, please clean your area including and not limited to sweeping the floor around your station before accepting another patron.
14.) Treat all patrons with respect and dignity.
15.) Do not chew gum, smoke or usevulgar language at any time.
16.) Remove all capes after you shampoo a patron and before placing under the dryer. Please make sure the heat is regulated prior to positioning your patron under the dryer.
17.) The student cannot leave school until all his/her patrons are combed out.
18.) Do not discuss your personal problems with the patron.
19.) Food and beverages are not allowed on the floor or classrooms.
20.) If you are not assigned a patron, you cannot lounge in the cafeteria or other areas of the school. You may chooss from

The following tasks when between patrons:


- Practice State Boards.
- Review Chapter questions in the State Board or Textbook.
- Complete assignments or projects requested by your instructor.
- Set and comb out a classic hairstyle or other as approved by your instructor.

21.) An Instructor must review student's work.
22.) Student's are not allowed to work on his/her own hair and nails.
23.) The clinic instructor will collect any cost for supplies used for personal use.
24.) Students will be charged full price for services rendered to family and friends.
2S.) No personal items can be stored in the patron area/Clinic.
26.) Work stations must be neat and clean at all times.
27.) All students must complete assigned cleanlng duties by day's end.
28.} Students cannot leave the clinic without permission from the instructor.
29.) Students are not allowed in the inventory/supply area with an instructor
30.) All students will be dismissed at the same time. (Do not leave separately)
31.) Constant violations of the procedures listed will result in termination.Go back


Conduct and dismissal policy
All students must adhere to PSBC standards of conduct and guidelines. Students are expected to conduct themselves in a mature and professional manner at all times. If a student's conduct is deemed inappropriate and/or dangerous to other persons the student is subject to suspension and/or termination. All disciplinary actions taken by PSBC will be given in written form in person or by mail to the student involved.

Refunds/Terminations/cancellations
federal Government and NACCAS compliance states that a student will be terminated within thirty - {30} days of the last physical day of dass attendance. enrollment is classified as the time between the actual start date and the date on which the student terminates enrollment per the set guidelines listed below. The last day physically attended in school by the student will constitute termination date. Absences and hours attended will be calculated in this event.

1.) A student has the option to cancel hisjher enrollment agreement at PSBC within 72 hrs. (3 days) after signing.

2.) In the event that a student's application is rejected by PSBC, all monies will be refunded excluding the non-refundable registration fee, within 5 business days after signing.

3.) Cancellation is deemed by the "date the written notification is received in " person at PSBC or by the postmark date on the envelope, if mailed.

4.) Any student who withdraw/terminates after 5 business days after classes begin, will be charged for the cost of their kit, mannequins and textbooks in addition to the following:

First and Second Term Termination:

PSBC retains 100% after the 5th week
PS8C retains 70% after the 4th week
PSBC retains 500/0 after the 3rd week
PSBC retains 350/0 after the 2nd week
PSBC retains 20% after the 1st week
PS8C will not retain any tuition monies prior to 1 week or 5 business days

No prior or written agreement:

The failure of a student to notify the director in writing of withdrawal may delay refund of tuition due pursuant to Section 5002 of the Education law.

If a student does not return from LOA on the scheduled date, this is considered termination and the scheduled return date will serve as the termination date. the refund will be determined from the last date of physical attendance.

In the above referenced cases a refund will be mailed within 30 days.

The cancellation of a course will result In a full refund.

A full refund is given if PSBC Closes or ceases operations.

In some cases the refund may be greater if the Title IV accrediting agency refund policy results in a greater refund.

If the student secured a loan, PSBC is entitled to the full instructional cost upon disbursement or receipt of loan proceeds

All outstanding balances due PSBC will be assessed 15% interest rate per month until paid or acceptable payment arrangement is established and adhered to. In the event that payment arrangements are established, any lapse in payment will result in the15% interest rate being re-assessed to the students outstanding balance. A Certificate of completion will be issued to each student that completes the course curriculum and has met his or her full financial obligation to PSBC.

Right to Terminate
PSBC has the right to terminate enrollment and this agreement in the event of violations of the rules and regulations outlined in this pamphlet.

Right to Modify
PSBC reserves the right to modify and/or change the rules and regulations outlined in this pamphlet. In this event the student will receive a copy of the modified section and the student and a PSBC will sign a memorandum of understanding of new modifications representative.

The classroom teacher maintains the attendance roster. The attendance roster is kept at the school at all times. In the event that a substitute teacher is used, the substitute must initial the day's attendance.

Attendance will be taken in the following manner:

1. Approximately ten minutes afterclass begins

2. Approximately ten minutes before dass ends

If absent a first, second, and third time, that student will be contacted by phone and the results will be recorded on the Absence/Drop Record form.

If absent a fourth and fifth time, the student will be contacted by telephone and In writing and will be advised of possible consequences. A copy of the same letter will be given to the student the next time the student comes to dass, at which time the student must sign the Absence/Drop Recordform acknowledging receipt of the letter.

If absent a sixth time, the student will be informed in writing that the course must be repeated and the student will be subject to paying additional tuition for the repeat The student will not be able to repeat more than one course per program due to absences.

Measures Taken When a Student Drops
If a student Is absent three consecutive times, without notifying the school, the student will be considered dropped. The school will contact the student by telephone and in writing and formally advised the student of this status. The . school will then request that the student respond in writing and provide reasons for withdrawal. The results will be recorded in the Absent/Drop Record form. Ifappropriate, the student will be informed that a leave of absence status is available. The student will also be informed that the tuition paid will remain valid.

Complaints and Grievances
All students or employees of a licensed Private or Registered Business School in NYS has the right to file a complaint with the NYS education Department .

Complaints may relate to standards, procedures, dass instruction, operations, advertising' practices, registration, tuition collection, charges/rates and/or students. In addition, complaints may indude and is not limited to faculty, management, teaching/methods, and financial and other complaints relating to your education at PSBC. full disclosure of information and material will be made available.

Tuition Reimbursement Fund
The pamphlet" What you should know about licensed Private Schools and Registered Business Schools In New York State explains the Tuition Reimbursement fund. The Fund is designed to protect the student's financial interest of students attending proprietary schools. If a school doses while you are l in attendance prior to the completion of your education program, then you may be " eligible for a refund of all tuition expenses which you have paid. Addendum "1" is attached for your information and how to file a complaint.

PSBC has a refund and cancellation policy as required by law. Please make sure that you review our policy and ask your instructor or counselor if you have any questions and/or concerns. If you are still not satisfied, please schedule a meeting with our Director.

The student should be aware that some informatlon in the catalog may change. It is recommended that students considering enrollment check with the school director to determine if there is any change form the information provided in the catalog. In addition, a catalog will contain information on the school's teaching I personnel and courses/curricula offered. Please be advised that the State I Department of Education (SDE) licenses all teaching personnel and independently approves all course/curricula offered. Therefore, it is possible that the course/curricula listed in the schools catalog may not be approved at the time that a student enrolls in the school or the teaching personnel listed in the catalog may have changed. It Is again recommended that you check with the school director I to determine if there are any changes in the courses/curricula offered or the I. teaching personnel listed in the catalog.
