Proof of age is required when enrolling. Acceptable forms of proof of age are birth certificates, driver's license, passport and any other proof of as deemed appropriate and approved by the Director. </span><br /><br /><span class="post_comments"><strong>High School Diploma or GED:</strong></span><br /><span class="post_comments"><strong></strong></span><br /><span class="post_comments">High School transcripts or GED Is required for proof of education.</span><br /><br /><span class="post_comments"><strong>Transfer Students:</strong></span><br /><br /><span class="post_comments">All transfer students to PSBC must submit an official transcript to the admissions office. The admissions team will evaluate all transcripts. At least 10% of the hours required for the certificate program must be completed at PSBC. In addition a practical and written examination will be administered. Upon approval of the transfer credits to PSBC by the NY State Education Bureau of Proprietary School Supervision, the new enrollee may register for his/her classes. Students who attended other cosmetology or technology school, college or university must complete an enrollment application. PSBC is currently ineligible to accept state and federal education grants and loans. However, during the application process, eligible students may make arrangements with the P5BC Director to pay tuition through monthly installment plans. During the interview, prospective students will receive information regarding tuition, school policies, tour of the facility and fill out an application for admissions. <br /><br />If a prospective student would like to enroll and is accepted into the school, the applicant will sign the Enrollment Agreement and receive a copy after a start date is agreed upon and the $100.00 registration fee is paid. Proof of age, and High School Diploma required. Any person seeking admission should be aware that the course requires a high level of manual dexterity and coordination and that there are extended periods of required stand up practical work required. Practical evaluation takes into consideration performance and . production. These grades are recorded on Progress Reports which are computed weekly.</span><br /><br /><span class="post_comments">To remain In good academic standing, a student must maintain at least a 70% average and complete the course in the designated time frame. A student who falls below a 70% average will have a conference with the Director before continuing the course. Any student failing to maintain at least a 70% average \n the academic and practical segments Is subject to probation. During probation the student's performance will be monitored and additional academic assistance will be given in an effort to assist the student to be removed from probation, the student must correct the deficiency that caused the probation and must do so before the next evaluation period. evaluations will be conducted 30 days following the last grading period. Failure to improve during a probationary period will subject the student to dismissal. All requests for LOA must be in writing and submitted to the Director's office. LOA are available from the administrative staff and are granted at the discretion of the Director. If approved, the student will not accumulate absent hours or received credit during the LOA. A LOA consists of a minimum of one week and a maximum of 60 days. A student is allowed one leave of absence per 12-month period. All absences must be reported to the office with explanation for absence . (Supporting documentation i.e. Doctor's slip etc. May be requested</span><br /><span class="post_comments">3.) Emergency calls as qualified by PSBC can be made at designated phones.</span><br /><span class="post_comments">4.) Cell phones and pagers are to beturned off during class (Okay for use during breaks).</span><br /><span class="post_comments">5.) Person hygiene and good grooming must be maintained in the classroom and clinic including use of deodorants, light fragrances and fresh breath.</span><br /><span class="post_comments">6.) Children, friends and/or relatives are not allowed in the facility during class sessions.</span><br /><span class="post_comments">7.) Seniors must be prepared for State Board as announced by management, with their own supplies.</span><br /><span class="post_comments">8.) The intake staff assigns patrons, you are not allowed to switch or refuse patrons.</span><br /><span class="post_comments">9.) Please exercise ethical code of standards and hospitality when greeting patrons.</span><br /><span class="post_comments">10.) Please greet your patron (pleasantly), make sure they are comfortable, draped secured properly and all necessary prep work is implemented.</span><br /><span class="post_comments">11.) Please review the customer slip for services and review with patron.</span><br /><span class="post_comments">12.) Outside products brought in by the patron cannot be administered at PSBC.</span><br /><span class="post_comments">13.) Upon completion of your patron, please clean your area including and not limited to sweeping the floor around your station before accepting another patron.</span><br /><span class="post_comments">14.) Treat all patrons with respect and dignity.</span><br /><span class="post_comments">15.) Do not chew gum, smoke or usevulgar language at any time. </span><br /><span class="post_comments">16.) Remove all capes after you shampoo a patron and before placing under the dryer. Please make sure the heat is regulated prior to positioning your patron under the dryer.</span><br /><span class="post_comments">17.) The student cannot leave school until all his/her patrons are combed out.</span><br /><span class="post_comments">18.) Do not discuss your personal problems with the patron.</span><br /><span class="post_comments">19.) Food and beverages are not allowed on the floor or classrooms.</span><br /><span class="post_comments">20.) If you are not assigned a patron, you cannot lounge in the cafeteria or other areas of the school. An Instructor must review student's work.<br />22.) Student's are not allowed to work on his/her own hair and nails.<br />23.) The clinic instructor will collect any cost for supplies used for personal use.<br />24.) Students will be charged full price for services rendered to family and friends.<br />2S.) Absences and hours attended will be calculated in this event.</span><br /><br /><span class="post_comments">1.) A student has the option to cancel hisjher enrollment agreement at PSBC within 72 hrs. (3 days) after signing. </span><br /><br /><span class="post_comments">2.) In the event that a student's application is rejected by PSBC, all monies will be refunded excluding the non-refundable registration fee, within 5 business days after signing. </span><br /><br /><span class="post_comments">3.) Cancellation is deemed by the "date the written notification is received in " person at PSBC or by the postmark date on the envelope, if mailed. </span><br /><br /><span class="post_comments">4.) The attendance roster is kept at the school at all times.
